Neighborhood Medical Centers, Inc. is a small chain of medical centers located in a medium-sized city. The company currently has four medical centers located in each of the quadrants of the city. The centers provide medical care for minor emergencies and for patients without a regular family physician who need occasional medical care. The centers fill a middle ground between traditional family physician practices and hospital emergency rooms. The centers do not make appointments. Patients are treated on a walk-in basis. The centers are open 24-hours a day, 7 days a week.

Each of the four medical centers has a unique Center ID number (1, 2, 3, or 4) assigned to it. All centers also have a name (e.g., Northwest Neighborhood Medical Center), address, and phone number. Each medical center is managed by one administrative staff member.

Each center has employees. Each employee falls into one of two distinct categories: administrative staff member or nurse. The company records the following information about each of its employees: employee ID (a unique number assigned by Neighborhood Medical Centers, Inc.), social security number, name (first name, middle initial and last name), home address, home phone number, and the center at which the employee works. No employee works at more than one center.

Specific additional information is recorded for employees in each of the employee categories. The medical centers record the office number and office phone number of each administrative staff member. The medical centers record the license number and license expiration date for each nurse.

In addition to the Neighborhood Medical Center employees, the company also requires the services of contract physicians. These physicians are not considered to be employees.
