On the behalf of SERVISTA Inc., I am pleased to offer you employment opportunity as Network Engineer. Upon Acceptance, your salary will be $65,000 per year. As per discussion, your start date is October 1st, 2015

- Work Schedule: Employees are expected to work full time, but not to exceed, 40 hours per week or as instructed by manager
- Compensation & Benefits: Employees get paid on monthly basis and payment schedule depends on employee’s start date. Employees are also entitled to worker’s compensation insurance, medical, dental, and vision insurance benefits also
- Employment Eligibility Verification: The Immigration Reform and Control Act of 1986 require all individuals employed after November 6, 1986, to establish their identity and eligibility to work in the United States. Therefore, it is mandatory for employees to complete I-9 (Employment Eligibility Verification Form) and present evidence of work authorization and identification documentation which shows employee’s entitlement of legal right to work in the United States of America
- Work Authorization: Employees are required to possess current and valid work authorization during employment with SERVISTA Inc. Employees on OPT & CPT work permits are required to make arrangements with educational institutions, in a timely manner, in order to obtain or renew work permits. It is employee’s responsibility to provide us copy of renewed work permit to continue employment and avoid any issues.
- Employment Agreement: Employees are required to understand and sign employment agreement upon acceptance and agreement specifies detailed employee’s rights and responsibilities during employment with SERVISTA Inc.
