b ) The two errors that occurred are the indicators of the needs of implication of managing and improving the treatment process. The two errors are:
- Error 1 occurred when the surgeon fail to forward an order to the hospital in regards to employ a needle locator to identify the location of tumor.
- Error 2 is in the pathology report, stating that the size of tumor was 1.6mm where in fact it was 1.6cm.
2. a) Appointment, mammograms and pre admit should be standardized, as both processes generally go the same way for every patient.
b) Surgery, report and consult should be artistic as it differs for every patient.
3. In doing the procedures, before starting, the parties should have completed all the paperwork required in order and it should be checked by another party to avoid any error. Giving trainings to each parties involved about the procedures should improve their performance and prevent problems like that to occur in the future.
