When UCIL was facing losses, they started cutting on expenses rather than scaling down operation, whichresult in massive excess off MIC. Even after having numerous accident before disaster, they didn'tcalculated the possible outcomes.

Accidents before disaster resulted in a loss of life and permanent damage to employee, yet they neglegtedsafety standards. Whistle blowers were avoided or transferred. Human resource were treated no more thanjust a disposable asset.

Basics of factory management and daily routine were avoided, which were specifically designed to avoidunwanted circumstances. Pipes were not disconnected, excessive storage of MIC than required, nonreplacement of faulty equipment….and the list goes on.
