DISCUSSING A TREATMENT PLAN
During the initial meeting with a client, the case manager will prepare a tentative treatment plan. The treatment plan would be constructed based on the interview and assessment of the client. During the evaluation process, the case manager and client will work together to determine the problem and a plan. The treatment plan will consist on the diagnosis, a list of goals the client wants to achieve, and the services needed to achieve those goals.
Case Details
Larry McCune is seeking help with a few problems that are interfering with him living a health and stable live. McCune was involved in a severe traffic accident at which his 4 year old daughter was killed (Summer, 2012). Since the accident, Mr. McCune experiences nightmares, headaches, and fears all modes of transportation. Mr. McCune has also developed a rather short temper with his co-workers so he hasn’t worked consistently. Mr. McCune’s marriage ended shortly after the accident and he doesn’t see his 9 year son as much as he would like (Summer, 2012). Mr. McCune deeply wants to get treatment for his problems so he can get back to work, be a better parent, and be social.
Details of the Assessment
The interview and assessment process shows that the client is suffering from grieve, depression, and anxiety issues due to his daughter’s death and a direct result of the accident. Mr. McCune needs further medical evaluation for neurological issues that may be causing him sleepless nights, being irritable, and being fearful of all forms of transportation. Counseling and support groups would be beneficial to him. Interacting with other people with similar situations would help him with his grieve.
Treatment Plan
The client and case manager work together to prepare a treatment plan and finalized during the service meeting. While developing a treatment plan, the case manager individualize the plan to specifically address the client’s needs (Summer, 2012). Mr. McCune has decides the goals he feel needs work are mental health, family and medical.
