Case Management Overview Paper

A case manager plays a very important role not only with clients, but also within the community. There are many roles and responsibilities that a case manager takes on in order to meet the needs of everyone, and being able to identify problems and causes of behavior using theoretical models is vital when it comes to serving the client and community.

A case manager has many roles and responsibilities. Some roles of a case manager are; assessing, planning, implantation, and refer the client to the appropriate and supportive services. The assessment consist of making initial contact with the client, identify the problem/problems, and gather and assess all the information given by the client. The planning process is when the case manager develops a complete picture of the client, develop an actual plan, and arrange for the appropriate services. The implementation process is coordinating/providing services, find a resolution for the problem, and monitor those services to assure the services are meeting the client’s needs. It is also important that the case manager refers the clients to the most supportive and appropriate services, because case managers are there to help the clients live a better lifestyle and to help them with their needs. Some responsibilities of a case worker are; be compassionate towards the client, figure out what their needs are, find and refer them to the correct services, make plans for treatment, collaborate with other professionals, and monitor the client’s progress being made. Being compassionate towards the client is important for a case worker, because in most cases those clients are in dire situations looking for help. By being compassionate, empathetic, and positive towards the client this could help them build up their confidence they need to keep pushing forward towards their goal. Figuring out what the client needs is important as well because then the case manager can start the referral process, and make plans for treatment. With effective collaborating defragmentation is less likely to happy, which makes it easier to achieve goals. When the case manager monitors the client they will be able to find out if the client is making progress or turning back to their old habits. It is important to track the client’s progress because it lets the case manager know what services are meeting the needs of the client and what needs are not being met.
