1) All the 32 thermowells of VDU heater (F102) outlet pass temperatures: Procured thermowells 3”x300#; actual process flange rating 1.1/2”X300#
Nearly 35 more thermowell mismatch we have observed with respect to process flanges in various circuits mainly in VDU column/related cooling water pipelines.
2) All the four numbers of mass flow meters of F101 and F102 fuel oil mass flow meters have been not heat traced. We have been forced to resend these flow meters to YIL for external jacket provision.
3) Bearing cooling tower package: Many instruments were not considered in point database.
4) Till date we have not received cause and effect drawing from EIL.
5) H2S detector with range of 50-500 ppm were procured (Nearly 6 no’s) whereas our safety alarm limit is only 5 to 10 ppm. Not able to calibrate these sensor in allowable range of 0-10 ppm.
6) Control valves were not painted according to type of valve : FC & FO valves ( Red & Green)
7) Nearly more than 80 numbers of pressure gauges were missed from procurement. BPCL initiated procurement action, but material yet to be received.
8) 2 no’s of rupture disc and 2 no’s of rotameters not ordered. BPCL placed separate Purchase order
9) Instrument point database was incomplete: Many range mismatch, alarm/trip value mismatch has been pointed out to EIL.
10) Orifice blind plates were procured with flanges initially and orifice plates ordered separately: Wastage of BPCL money
11) For D-D/2 orifice tappings, orifice flanges were not procured as plate has to be installed in process flanges. But orifice plates were procured with 300# rating whereas process flange rating is 150#. We have been forced to machine orifice plates to match process flanges.
12) For viscosity and density analysers requirement of process inlet connectivity and analyser return connection back to process is not passed on to piping and it is not shown in isometric drawings.
13) Cooling water supply, cooling water return and utility requirement like instrument air/Nitrogen for analysers is not passed on to piping and it is not reflecting in isometric drawings.
14) In both CDU-4 and SWS-4 CBD vessels: Radar gauge flange rating was not matching with vessel flange rating and SS Stillwell was not procured along with radar gauge.
